Patients on Pregabalin and Gabapentin


As from now, Pregabalin and Gabapentin are being classed as controlled drugs. This means that:


1. Prescriptions will not be able to be sent electronically to your nominated pharmacy



  • 2.  Prescriptions have to be collected from the surgery as they require a signature by the patient, a relative or a pharmacy

  • Maximum quantity allowed to be issued is 28 Days


If you feel you may want to reduce or stop either of these medications please make an appointment at the surgery to discuss with one of the Practice Pharmacists.

NEW PATIENTS

registrationAll new patients joining the practice will be asked to complete a registration form to apply to join the practice.

CHANGE OF PERSONAL DETAILS

If you change your NAME, ADDRESS or TELEPHONE NUMBER, please give us full details, including your postcode. Remember to tell us if details for other members of the household have also changed. We need to keep up to date information on the computer and notify the Health Authority.


MOVING OUT OF THE AREA

Unfortunately we have to say goodbye to patients moving from our catchment area. Please register with a new GP as soon as possible so that the Health Authority can transfer your medical records.
